Provide access to your IdentityIQ account so DSP can evaluate your credit report. Only submit an account that belongs to you. Do not provide your full Social Security number.

Your submission is encrypted in storage and accessible only through Lou’s private review area. Credentials are not included in email alerts or sent to Capsule. Records expire after 30 days and are removed when the intake or review area next processes a request. Contact DSP to revoke authorization; change your IdentityIQ Password to end account access.
